New Patient Registration

The Doctors welcome new patients within our practice area. Any patient wishing to register can check if their address is within the catchment area - their address must be within an 8 mile radius.

Patients transferring GPs within Northern Ireland/GB should complete the following forms:

HS200 Registration Form + photographic ID

Stream Street Surgery Registeration Form (collect from reception desk)

Stream Street Surgery Application Form (collect from reception desk)

Patients arriving in Northern Ireland from outside the UK should complete the following forms:

HSCR1 Registration Form + provide relevant forms of ID (details are available on this form)

Stream Street Surgery Registeration Form (collect from reception desk)

Stream Street Surgery Application Form (collect from reception desk)

 

Please return your completed registration forms and health questionnaire to the Surgery along with all required documentation so that your registration can be processed.

New Babies

You should register the birth of your baby immediately with the Registrar of Births. You will then be given a yellow form (Form HS 123) which you should complete and hand to the Doctor’s receptionists as soon as possible.

Please ensure that we have an up-to-date telephone number on our records. If you change your name, address or telephone number please let us know at reception as soon as possible.
